Our Mission

Our mission is to be a Trusted for Successful Therapy in the Communities We Live. Our first clinical location opened in Aylmer in 2002. Since then, we have expanded to a total of 6 physiotherapy clinics, located throughout Elgin and Kent Counties.

Our Vision

Our vision is to provide patients who live in smaller communities the ability to access quality physiotherapy and musculoskeletal services locally, and prevent the need to travel to larger cities for care.

Injuries are as individual as the person who has them. We treat each person with an individualized treatment plan in order to help them achieve their maximum level of improvement.

Our History

Physiotherapist/Owner Chris Streib opened the first Talbot Trail Physiotherapy clinic in his hometown of Aylmer in 2002, with the goal of being able to provide quality health care to those communities who were underserviced, and residents needed to go to bigger cities for treatment.

With a mission to make Talbot Trail Physiotherapy, “Trusted for Successful Therapy in the Communities We Live”, Chris built on his patient centred physiotherapy philosophy, and expanded from Aylmer into St. Thomas, West Lorne, and Blenheim in order to provide physiotherapy to the residents of these communities.

Chris entered into a collaborative agreement with the St. Thomas Elgin General Hospital in the spring of 2008 to reopen the 22,000 sq. ft. rehabilitation centre. This location would become a Centre of Excellence, utilizing the most progressive treatment options and state-of-the-are equipment, including an indoor 95 degree saltwater pool to ensure the best possible options for patient recovery.

In 2011, clinics were also added in West Lorne and Blenheim. In 2018, a third location was added in St. Thomas, now located at 1025 Talbot St, providing a variety of treatment locations/options for the city and surrounding area.

Yes, we can bill directly to a large number of insurance companies (for most policies). To ensure we have the correct data, please bring your benefits card with you as we will need the policy holder’s name, DOB, policy # and ID/certificate #.

Yes, if you have been involved in a motor vehicle accident (MVA), please ensure you have completed and submitted your OCF 1 to your insurance company PRIOR to attending your assessment. You will then be requested to provide your policy #, claim #, date of accident, Drivers Licence # as well as the name, address and phone number of your motor vehicle adjuster and insurance company.

 

Yes, if you have been involved in a workplace injury, we can bill directly to WSIB. You will need to ensure that you have completed all necessary paperwork with the Emergency Doctor or your Family Physician PRIOR to attending your assessment. You will then be requested to provide your WSIB Claim #, Date of Injury, employers name, address and phone number as well as your WSIB adjuster (if you know it).

Yes, if you have had hip, knee or shoulder replacement (where applicable) surgery done, and you fall under the Bundled Care Program, we can bill the hospitals directly. You will be provided with a package from the Hospital that will direct you further.

Yes, our Blenheim location is the ONLY one that offers OHIP funded services. You will need to fall under the current OHIP criteria to qualify. These questions will be asked of you when you call our Blenheim location to book your OHIP assessment.

Physician referrals are not required for physiotherapy; however, most extended health benefits require it for submission, as well as OHIP (in select locations) require it for funding.

We ask that you arrive 15 minutes before your scheduled Assessment time. Bring a short sleeved t-shirt/tank top or shorts to change into for the assessment if needed.

If you are taking any medications, please bring a list with you, along with why you are taking them.

Your account will be settled at each visit, and we do accept a variety of payment options including: Cash, Mastercard, Visa and Debit Card.

Please notify the clinic 24 hours in advance of your appointment if you are in need to cancel, as this rehabilitation time has been reserved for you. A cancellation fee will be administered if less than 24 hours notice is given.
